Adult Community Services, Age 21 and Over

Community Crossroads contracts with a variety of vendors to provide both residential and day services as well as supported employment and other services to adults with disabilities and acquired brain injuries. We work in partnership with individuals, their families/guardians, area agency staff, subcontractors and the community to plan, coordinate, facilitate, implement and monitor services and supports for individuals who qualify for the Area Agency system. The individual/family and the team will design an Individual Service Agreement that meets the needs of each individual and their family lifestyle. We advocate on behalf of the individual to ensure that all services are designed to promote the individual’s personal development, independence and quality of life in a manner that is determined by the individual and his or her guardian family.

Services can be provided in a variety of settings such as your own home, day program, job site, residential setting or other community setting.

Adult Community Services are provided through state and federal funds. Community Crossroads is committed to providing high quality services that meet the needs of those we serve.

Community Crossroads provides opportunities for individuals to be involved with our People Power group which is a self advocacy group that meets once a month to help adults work together by taking charge of their lives and make appropriate decisions that effect their lives in a positive way. We also have the Self Determination group which also meets once a month which is a fun and social opportunity to learn skills and information that will help each individual make plans and decisions about their lives. We provide educational opportunities for individuals who receive our services and supports.

We are very excited about employment opportunities for those we serve. Our vendors assist adults as desired to develop job skills, secure employment and become community volunteers.
