Registered Nurse (RN)

We are seeking a registered nurse to provide medical case management as part of a collaborative team with a variety of interdisciplinary professionals, caregivers, individuals, and family members. Primary functions include evaluating health risk assessments, providing nursing guidance and judgement with respect to care plans, functional needs assessments and medication administration oversite. This role will serve as a consultative support for the agency’s service coordinators and serve as a link with local community healthcare providers.

  • Primary schedule M-F 8:30-4:30

Family Support Coordinator

We are looking to add to our professional team of Service Coordinators. In this role, you will ensure the people we serve receive the quality services they need to be successful in their lives and in their communities. Ideal candidates will demonstrate excellent verbal and written communication skills, be highly organized, have an optimistic attitude, and demonstrate a commitment to the important work we do!

Responsibilities

  • Become familiar with community resources and connect individuals and families with the resources they need.
  • Advocate on an agency and statewide level for services and supports needed.
  • Ensure that individuals are given the necessary services they require, as well as following up with ongoing service needs.
  • Maintain good relationships with the families and individuals served, as well as vendor agencies.
  • Represent the agency in a professional manner.

 Qualifications

  • Bachelors in Human Services, Psychology, Social Work or related field.
  • 1-3 years of case management/rehabilitation or family support experience.
  • Excellent verbal and written communication skills.
  • Ability to multitask and meet deadlines.

Case Manager

Community Crossroads is a progressive non-profit Human Services agency that provides services to individuals/elders with complex medical needs and disabilities. We are seeking a highly motivated professional for the position of Case Manager in our Choices for Independence Program. An understanding of home care through a person-centered approached is desired.

This is an exciting opportunity to be part of a growing and changing organization. A Bachelor Degree in Human Services, Psychology, Social Work or related field is preferred. Previous experience in case management, rehabilitation or family support is helpful.
