PDMS Service Coordinator

Community Crossroad’s PDMS Department is ready to expand its team and hire a full time PDMS Service Coordinator. This position will carry a dedicated caseload of PDMS individuals/families and will report to the Director of PDMS Operations. Candidates for this position should possess excellent skills in written and oral communication, interpersonal skills, organization, and the ability to manage multiple priorities. Moreover, they should have an interest and willingness to assist and support those families who have chosen to direct and manage their own services rather than to have their family member’s services directed and managed by an agency or other third-party vendor /agency.

Case Manager

Community Crossroads is a progressive non-profit Human Services agency that provides services to individuals/elders with complex medical needs and disabilities. We are seeking a highly motivated professional for the position of Case Manager in our Choices for Independence Program. An understanding of home care through a person-centered approached is desired.

This is an exciting opportunity to be part of a growing and changing organization. A Bachelor Degree in Human Services, Psychology, Social Work or related field is preferred. Previous experience in case management, rehabilitation or family support is helpful.

Adult Service Coordinator

We are looking to add to our professional team of Service Coordinators. In this role, you will ensure the people we serve receive the quality services they need to be successful in their lives and in their communities. Ideal candidates will demonstrate excellent verbal and written communication skills, be highly organized, have an optimistic attitude, and demonstrate a commitment to the important work we do!

Responsibilities

  • Become familiar with community resources and connect individuals and families with the resources they need.
  • Advocate on an agency and statewide level for services and supports needed.
  • Ensure that individuals are given the necessary services they require, as well as following up with ongoing service needs.
  • Maintain good relationships with the families and individuals served, as well as vendor agencies.
  • Represent the agency in a professional manner.

Qualifications

  • Bachelors in Human Services, Psychology, Social Work or related field.
  • 1-3 years of case management/rehabilitation or family support experience.
  • Excellent verbal and written communication skills.
  • Ability to multitask and meet deadlines.
